DAAS-733 <br /> (Rev.2116) <br /> Home and Community Care Block Grant for Older Adults <br /> County Funding Plan <br /> July 1,2017 through June 30,2018 <br /> Methodology to Address Service Needs of Low Income(Including Low-Income <br /> Minority Elderly),Rural Elderly and Elderly with Limited English Proficiency <br /> (Older Americans Act,Section 305(a)(2)(E)) <br /> Community Service Provider: Cabarrus County DHS-Transportation <br /> County: Cabarrus <br /> The Older Americans Act requires that the service provider attempt to <br /> provide services to low-income minority individuals in accordance to their <br /> need for aging services. The community service provider shall specify how <br /> the service needs of low income, low-income(including low income <br /> minority elderly), rural elderly and elderly with limited English <br /> proficiency will be met through the services identified on the Provider <br /> Services Summary(DAAS-732). This narrative shall address outreach and <br /> service delivery methodologies that will ensure that this target population is <br /> adequately served and conform with specific objectives established by the <br /> Area Agency on Aging, for providing services to low income minority <br /> individuals. Additional pages may be used as necessary. <br /> Cabarrus County Transportations goal is to coordinate with teh Adult and Aging department to provide <br /> meal site and medical transportation to the senior population within Cabarrus County.This joint effort <br /> is based on need through both referall and inquiries.Cabarrus County Transportation will also <br /> participate in any outreach events open to us to promote transportation programs available to the <br /> senior population.In addition,Cabarrus County Transportation drivers are trained in passanger <br /> sesitivity,CPR,Bloodborne Pathogens,First Aid and Defensive Driving.Our drivers are also trained <br /> on signs of elderly abuse and the process in which to report it. <br /> Attachment number 1 \n <br /> F-4 Page 93 <br />
