DAAS-733 <br /> (Rev.2116) <br /> Home and Community Care Block Grant for Older Adults <br /> County Funding Plan <br /> July 1,2017 through June 30,2018 <br /> Methodology to Address Service Needs of Low Income(Including Low-Income <br /> Minority Elderly),Rural Elderly and Elderly with Limited English Proficiency <br /> (Older Americans Act,Section 305(a)(2)(E)) <br /> Community Service Provider: Department of Social Services <br /> County: Cabarrus <br /> The Older Americans Act requires that the service provider attempt to <br /> provide services to low-income minority individuals in accordance to their <br /> need for aging services. The community service provider shall specify how <br /> the service needs of low income, low-income (including low income <br /> minority elderly), rural elderly and elderly with limited English <br /> proficiency will be met through the services identified on the Provider <br /> Services Summary(DAAS-732). This narrative shall address outreach and <br /> service delivery methodologies that will ensure that this target population is <br /> adequately served and conform with specific objectives established by the <br /> Area Agency on Aging, for providing services to low income minority <br /> individuals. Additional pages may be used as necessary. <br /> Cabarrus County Department of Human Services goal is to increase participation in services <br /> and programs by low income elderly and elderly with limited English <br /> proficiency in the community. DHS provides assistance to all individuals <br /> living within the county's boundaries requesting services regardless of <br /> location within the county. The type and extent of services clients receive <br /> are based on clients'needs and availability of funding. Outreach by the agency has <br /> been accomplished by brochures,community forums,word of mouth and <br /> contacts with Community <br /> partners. Outreach is ongoing. Request from clients who meet the need for HCCBG services <br /> will continue to be accepted. New clients will be added as existing <br /> clients leave the program or experience a decrease of units needs or <br /> additional funds are secured. As an agency with multiple funding <br /> Sources,assessments will be made with each case to be sure that the most <br /> appropriate funding is used to best meet the needs of the clients and that <br /> the Older American Act funds are for the traget population <br /> groups. <br /> Attachment number 1 \n <br /> F-4 Page 105 <br />
