DAAS-733 <br /> (Rev.2116) <br /> Home and Community Care Block Grant for Older Adults <br /> County Funding Plan <br /> July 1,2017 through June 30,2018 <br /> Methodology to Address Service Needs of Low Income(Including Low-Income <br /> Minority Elderly),Rural Elderly and Elderly with Limited English Proficiency <br /> (Older Americans Act,Section 305(a)(2)(E)) <br /> Community Service Provider: Cabarrus County Planning&Development' <br /> County: Cabarrus <br /> The Older Americans Act requires that the service provider attempt to <br /> provide services to low-income minority individuals in accordance to their <br /> need for aging services. The community service provider shall specify how <br /> the service needs of low income, low-income(including low income <br /> minority elderly), rural elderly and elderly with limited English <br /> proficiency will be met through the services identified on the Provider <br /> Services Summary(DAAS-732). This narrative shall address outreach and <br /> service delivery methodologies that will ensure that this target population is <br /> adequately served and conform with specific objectives established by the <br /> Area Agency on Aging, for providing services to low income minority <br /> individuals. Additional pages may be used as necessary. <br /> The Planning&Development's goal is to coordinate and provide Housing and Home Improvement <br /> Services in conjunction with the Department of Human Services that is funded by the Home and <br /> Community Care Block Grants.Coordination of this function will produce a service product that is both <br /> accountable and accessible by the full scope of the clients for which the HCCBG funds were <br /> designated.It is our intent to meet the needs of that specialized population. The Planning& <br /> Development Department will coordinate with the Department of Human Services and other service <br /> providers to ensure that the population is served. All eligible cleints will be provided access to service. <br /> The Planning&Development Department will follow all program guidelines for service provision and <br /> client treatment. Homes will be modified using program guideline eligibility for services such as <br /> handicap accessibility,door widenings,minor roof repairs,and safety improvements. Any <br /> improvements will be made in compliance with all program regulations. The Planning&Development <br /> Department will take referrals from the Department of Human Services and other service providers of <br /> the target population.Outreach is achieved through these programs,word of mourth and visiting <br /> nutrition sites and other programs designed to address the target population. <br /> Attachment number 1 \n <br /> F-4 Page 119 <br />
