Cabarrus County Department of Human Services <br />Waiting List Management <br />PURPOSE: This policy is designed to describe the process followed if applicants for the Home <br />Community Care Block Grant (HCCBG) (Adult Day Care/Health, Congregate Nutrition, <br />Consumer Directed Services, Family Caregiver Support, In -Home Aide Services), Social <br />Services Block Grant (SSBG) In -Home Aide Services, State Adult Day Care /Health Funding and <br />Senior Community Service Employment Program (SCSEP) in Cabarrus County are placed on <br />any waiting list. <br />POLICY: Cabarrus County has chosen to maintain a waiting list for the purpose of responding <br />to inquiries about the service requested. Eligibility of persons on the list is not determined until <br />resources to provide the service become available. Once resources become available, applicant's <br />situation will be reviewed, priority designation will be made based on specific program <br />guidelines and a determination of eligibility will be established. <br />NOTIFICATION: Letter (Appendix A) will be sent to applicant making them aware of a <br />waiting list and to provide contact information for any updates or questions. If services cannot <br />be provided within 90 days of request, applicants will be notified that requested service cannot <br />be provided yet . Thereafter they will notified bi- annually until services are provided or <br />applicant no longer needs service (Appendix B). <br />Attachment number 1 \n <br />F -3 Page 34 <br />
