DAAS -733 <br />Nome and Community Care Block Grant for Older Adults <br />County Funding Plan <br />Rev. 21 t 6) <br />Methodology to Address Service heeds of Low-income Minority Elderly and Rural Elderly <br />(Older American Act, Section 305 (a)(2)(E )) <br />Community Service Provider: Cabarrus County Active Living and Parks <br />County: Cabarrus <br />July 1, 2016 through June 30, 2017 <br />The Older Americans Act requires that the service provider attempt to provide services to low- income <br />minority individuals in accordance to their reed for aging services. The community service provider <br />shall specify how the service needs of low- income, low income (including low income minority <br />elderly), rural elderly and elderly with limited English proficiency will be met through the <br />services identified on the Provider Services Summary (DAAS -732). TNs narrative shall address <br />outreach and service delivery methodologies that will ensure that this target population is adequately <br />served and conform with specific objectives established by the Area Agency on Aging for providing <br />services to low- income minority individuals. Additional pages may be used as necessary. <br />To encourage participation in programs provided through the Cabarrus County Active Living and Parks <br />Department (and Senior Centers) for low income, minority and rural older adults in the community. <br />The Cabarrus County Senior Centers provide programs for all older adult residents regardless of race, <br />residence, or economic status. <br />Outreach efforts include: <br />e A PowerPoint presentation is used to provide outreach into all areas of the community. <br />o Approximately 6 copies of the printed news magazine are distributed three times per year and is <br />also available on the Cabarrus County website. <br />® Referrals are provided to the Human Services Department and received from various local providers, <br />o Newcomers packet is being developed to provide overview of programs to persons new to the area. <br />* Brochures are distributed throughout the community that promote programs provided by our agency. <br />o Programs are promoted through local churches and through speaking presentations to older adults <br />and local civic groups. <br />* Programs are provided to a variety of satellite locations (churches, recreation centers, YMCAs, <br />etc.) throughout the county, including rural and low- income areas. <br />mber 1 \n <br />F -6 Page 106 <br />
