Use as much space as needed to answer the questions.. <br />*Please return this form to Pepper Sego no later than August 19 2016. <br />Agency Name: Cabarrus County <br />Contact Person: Kelly Sifford 704- 920 -2142 <br />2015 -2016 HOME- ASSISTED ACTIVITIES <br />Proposed Activities (per Action Plan): <br />The 2012 -2013 funding was amended to complete one home reconstruction and one single <br />family owner Occupied rehabilitation project.. <br />2. Describe how you provided decent housing and a suitable living environment for <br />low and moderate - income persons and how it made an impact on identified needs. <br />Cabarrus County has previously identified low to moderate income elderly and disabled <br />homeowners with substandard housing as the greatest housing need in its jurisdiction. <br />Recent changes in the population are refocusing efforts toward low income elderly rental <br />units in the future. Low to moderate income persons and families are selected for the <br />single family rehabilitation after going through an application process. The homes are <br />inspected and evaluated for their needs. Qualified applicants have their homes <br />rehabilitated through the HOME program. Cabarrus County uses standardized inspection <br />forms and lead reports by certified inspectors to create work write -ups to meet all the code <br />needs and other needs as defined by the circumstances. Common rehabilitation activities <br />are: roof replacements, electrical and plumbing work, energy efficiency updates, windows, <br />doors, and handicap accessibility features. All of these activities work to keep the home <br />affordable and safe for the client. It also helps Cabarrus County keep clients in the home <br />when accessibility measures are taken. Most work is focused on low to moderate income <br />elderly and disabled clients, thereby meeting the published goals of the program. <br />Attachment number 1 <br />G -1 Page 166 <br />
