CABARRUS COUNTY <br />BOARD OF COMMISSIONERS <br />REGULAR MEETING <br />JULY 20 2015 <br />6:30 P.M. <br />AGENDA CATEGORY: <br />Consent Agenda <br />SUBJECT: <br />Human Resources - Approval to Amend a Contract with CHS to Provide <br />an Onsite Employee Health and Wellness Clinic <br />BRIEF SUMMARY: <br />Carolinas HealthCare System (CHS) was selected to provide services for <br />an onsite clinic for Cabarrus County employees, retirees and dependents. <br />In order to take the next step, an amendment to the existing contract with <br />CHS needs to be approved. Representatives from CHS will be available <br />for questions on scope of services and implementation plan for the new <br />clinic to be housed in the former location off of Church Street. <br />REQUESTED ACTION: <br />Motion to approve an amendment to contract 20150224 between <br />Cabarrus County and Carolinas HealthCare System to provide for a <br />dedicated onsite clinic and to authorize the County Manager to execute <br />the contract on behalf of Cabarrus County, subject to review or revision by <br />the County Attorney. <br />EXPECTED LENGTH OF PRESENTATION: <br />SUBMITTED BY: <br />Johanna Ray, Health and Wellness Manager <br />Lundee Covington, Human Resources Director <br />BUDGET AMENDMENT REQUIRED: <br />F -3 Page 64 <br />
