DOA-733 <br />Home and Community Core Block Grant for Older Adults <br />County Funding Plan <br />Methodology to Address Service Needs of Low-income Minority Elderly and Rural Elderly <br />(Older American Act, Section 3 OG(a)( 5 )(A)(ii) <br />Community Service Provider: Cabarrus County Transportation <br />County: Cabaruo <br />July 1, 2014through June 30, 2015 <br />The Older Americans Act requires that the service provider attempt to provide services to low-income <br />minority individuals in accordance 8z their need for aging services. The community service provider <br />shall specify how the service needs of low-income minority elderly and rural elderly will be met through <br />the services identified nn the Provider Services Summary (DDA'732). This narrative shall address <br />outreach and service delivery methodologies that will ensure that this target population ioadequately <br />served and conform to specific objectives established by the Area Agency on Aging for providing <br />services to|ow'income minority individuals. Additional pages may he used aynecessary, <br />GOAL: <br />CCTS' mission is to provide transporation servcies that enable all elig�le indviduals to access <br />necessary medical care and other resouces to improve and enhance their quality of life by providing <br />transportation for essential services. VVe promote independent lifestyles that allows individuals to <br />reaminin their homes oo long aspossible. <br />GOAL: <br />To increase our partiGipa�on beyond our medical transportation responsibilities wKch will include more <br />service to the elder|y and handicapped which will hopefully increase quality of ffe for these individuals. <br />Attachment number 2 <br />F-S Page 102 <br />
