Pleasant -$ L a . 0 0 <br />• Out of Cabarrus County - $ <br />CCTS requires transportation requests to be cancelled at least two hours prior to pick up. Any <br />cancellation requested fewer than two hours prior to pick up may be subject to the no -show rate of <br />$ for any mileage accrued in the attempt. <br />It is the intent of the proposed contract to allow for contract price changes only when the unleaded <br />gasoline price changes in 30 cents increments from the base price. The base price will be established <br />as the average price of unleaded gasoline listed on the North Carolina Department of Administration's <br />Division of Purchase and Contract's Term Contract for Gasoline (Transport) website <br />(http://www.doa,state.nc.us/PandC/4051 Ssl. htm). <br />For each increase or decrease of 30 cents in gasoline from the established base price, the contract <br />price may increase or decrease by up to 15 cents. The effective date of any contract price change will <br />be the first day of the following month when the website listed above reflects an incremental change <br />from the established base price. Per trip baselines will be adjusted per cumulative trip miles. <br />Independent Price Determination: The Respondent certifies by submission of a response to this RFP <br />that it has not colluded, conspired, connived, or agreed, directly or indirectly, with a Proposer or person <br />to refrain from proposing, and further that it has not in any manner, directly or indirectly, sought by <br />agreement, collusion, communication or conference, with that of any person, to fix the proposal amount <br />herein or that of any other Respondent, or to fix the agreement amount herein or cost element of said <br />agreement amount, or that of any other Proposer, or to secure any advantage against Cabarrus County <br />or any other Proposer, or to secure and advantage against Cabarrus County or any person interested <br />in the proposed contract. <br />Payment to Contractor <br />Attachment number 2 <br />F -4 Page 110 <br />
