Ms. Fulton has served <br />one year and does not wish to serve another term. It is requested to <br />remove her name from the committee roster. <br />Mary Stewart's term of appointment on the Adult Care Home Community <br />Advisory Committee ends October 31, 2014. Ms. Stewart has served one <br />year and does not wish to serve another term. It is requested to remove <br />her name from the committee roster, effective October 31. <br />An application was received from La Shonda Sparks to serve on the Adult <br />Care Home Community Advisory Committee. She has been prequalified <br />by Laurie Abounader, Regional Ombudsman, and is recommended for an <br />initial one -year appointment. <br />REQUESTED ACTION: <br />Motion to remove Dora Fulton and Mary Stewart from the Adult Care <br />Home Community Advisory Committee roster and thank them for their <br />service. <br />Motion to appoint La Shonda Sparks to the Adult Care Home Community <br />Advisory Committee for a one -year term ending October 31, 2015. <br />EXPECTED LENGTH OF PRESENTATION: <br />H -1 Page 267 <br />
