May <br />positive drug or alcohol test results, documentation of <br />49 CFR 655.71 <br />include test results, evaluations and referrals, annual <br />refusals to take required alcohol and /or drug tests <br />summary reports, education and training records, <br />(including substituted or adulterated drug test results), <br />chain of custody forms, and all other program related <br />referrals to Substance Abuse Professionals (SAP), SAP <br />documents. <br />reports, all follow -up tests and schedules for follow -up <br />tests, copies of annual Drug & Alcohol Management <br />Information System (MIS) reports submitted to Federal <br />Transit Administration (FTA), equipment calibrations, and <br />records related to the administration of the testing <br />program after 5 years. <br />b) Destroy in office records obtained from previous <br />employers concerning drug and alcohol test results after 3 <br />years. <br />c) Destroy in office records of the inspection, maintenance, <br />and calibration of Evidential Breath Testing Devices (EBTs), <br />records related to the collection process, and records <br />concerning the training of program staff after 3 years. <br />d) Destroy in office records of negative and cancelled drug or <br />alcohol test results, including alcohol test results with a <br />blood alcohol concentration of less than 0.02, after 1 year. <br />Retention Note: Records should be maintained in a location <br />with controlled access. <br />*See AUDITS, LITIGATION, AND OTHER OFFICIAL ACTIONS, page vi. <br />t See signature page.
