5.11 Industrial Waste Survey <br />(a) At a frequency established by WSACC, an industrial waste stit'vey that identifies industrial users and <br />characterizes their discharge of wastewater will be conducted. Each user included in the survey is required to <br />respond with complete and accurate infor €iaation on the specified schedule. <br />(b) The survey results may be used to establish an industrial user inventory and to modify or issue any applicable <br />wastewater discharge permit. <br />(c) Failure to respond to the survey completely and accurately may result in any enforcement action that WSACC <br />may determine to be appropriate. <br />5.15 Forms <br />WSACC may establish such forms as the Executive Director determines to be appropriate and require the use of such <br />forms in the submission of any report, application, request, or other information contemplated by this ordinance. Failure <br />to use the established farm for the submission of a report, application, request, or other information may result in the <br />rejection of the Submission. <br />5.16 Electronic Reporting <br />The [POTW Director] may develop procedures for• receipt of electronic reports for any reporting requirements of this <br />Ordinance. Such procedures shall comply with 40 CFR Part 3. These procedures shall be enforceable tinder Section 8 <br />of this Ordinance. <br />5.17 Special Resorting Recinirements for lUs in Satellite POTWs <br />In the case of industrial user located in a Satellite POTW organization's jurisdiction, all information required to be <br />reported to the industrial tiser's Pretreatment Program Control Authority by the Section may also be required to be <br />reported to the POTW treatment plant organization. <br />SECTION G - COMPLIANCE MONITORING <br />6.1 Monitoring Facilities <br />(a) WSACC requires the user to provide and operate at the user's expense, monitoring facilities to allow <br />inspection, sampling, and flow measurement of the building sewer and/or internal drainage systems. The <br />monitoring facility should nor•tnally be situated on the users premises, but WSACC may, when such a location <br />would be impractical or cause undue hardship on the user, allow the facility to be constructed in the public <br />street or sidewalk area and located so that it will not be obstructed by landscaping or parked vehicles. <br />(b) There shall be ample room in or near such sampling manhole or facility to allow accurate sampling and <br />preparation of samples for analysis. The facility, sampling, and measuring equipment shall be maintained at all <br />times in a safe and proper operating condition at the expense of the user. <br />( c) Whether constructed on public or private property, the monitoring facilities shall be provided in accordance <br />with WSACC's requirements and all applicable local construction standards and specifications. Construction <br />shall be completed within 90 days following written notification by WSACC. <br />(d) Should WSACC determine that a monitoring facility does not comply with this ordinance, or that access <br />thereto has been denied, WSACC may require the user, at its own expense, to modify or relocate the <br />93 <br />Attachment number 7 <br />H -3 Page 298 <br />
