SECTION IV: REPORTING AND COMPLIANCE 1. Provider shall provide the appropriate representative with the necessary credentials to attend all client staffings, court sessions as necessary, senior advisory committee meetings, conferences and training seminars. 2. Provider shall be required to provide on a monthly basis, client specific information to be utilized by the court to monitor treatment compliance and produce reliable outcome measures. The provider shall collect at a minimum the following data: • Number of clients assessed • Number of clients entering treatment • Number of clients obtaining each level of treatment and the number of hours of treatment per client • Number of clients referred to alternative treatment programs • Number of clients completing treatment and /or each phase of treatment • Number of clients exiting treatment and means /reasons for termination 3. Provider shall comply with all applicable provisions of the Criminal Justice Partnership Act and complete and submit all necessary forms, data entry and /or reports required. 4. Provider shall make all treatment documentation /files available for review as necessary. 5. The services delivered by the Provider under the terms of this contract shall meet the minimum standards of providing substance abuse treatment services set forth by the North Carolina Department of Health and Human Services (DHHS). 6. Provider will comply with the appropriate inspection, licensing, and zoning required by the State Fire Marshall's office, as well as other applicable local regulations. 7. Provider shall abide by all laws and regulations of the State of North Carolina and shall hold appropriate licenses necessary to provide substance abuse treatment. 8. Provider understands that the services of this contract cannot be subcontracted or assigned to other providers without written consent of the County. Attachment number 1 F -4 Page 69
