SECTION IV: REPORTING AND COMPLIANCE <br />1. Provider shall provide the appropriate representative with the <br />necessary credentials to attend all client staffings, court sessions as <br />necessary, senior advisory committee meetings, conferences and <br />training seminars. <br />2. Provider shall be required to provide on a monthly basis, client specific <br />information to be utilized by the court to monitor treatment compliance <br />and produce reliable outcome measures. The provider shall collect at a <br />minimum the following data: <br />• Number of clients assessed <br />• Number of clients entering treatment <br />• Number of clients obtaining each level of treatment and the number <br />of hours of treatment per client <br />• Number of clients referred to alternative treatment programs <br />• Number of clients completing treatment and /or each phase of <br />treatment <br />• Number of clients exiting treatment and means /reasons for <br />termination <br />3. Provider shall comply with all applicable provisions of the Criminal <br />Justice Partnership Act and complete and submit all necessary forms, <br />data entry and /or reports required. <br />4. Provider shall make all treatment documentation /files available for <br />review as necessary. <br />5. The services delivered by the Provider under the terms of this contract <br />shall meet the minimum standards of providing substance abuse <br />treatment services set forth by the North Carolina Department of <br />Health and Human Services (DHHS). <br />6. Provider will comply with the appropriate inspection, licensing, and <br />zoning required by the State Fire Marshall's office, as well as other <br />applicable local regulations. <br />7. Provider shall abide by all laws and regulations of the State of North <br />Carolina and shall hold appropriate licenses necessary to provide <br />substance abuse treatment. <br />8. Provider understands that the services of this contract cannot be <br />subcontracted or assigned to other providers without written consent of <br />the County. <br />Attachment number 1 <br />F -4 Page 69 <br />
