FACILITY COMPJ,IANCE AUDIT REPORT <br />Division of Waste Management <br />Solid Waste Section <br />Page 2 of 2 <br />AREAS OF CONCERN AND COMMENTS: <br />1. The modification to the Permit to Operate included the operation of the East Section of Phase 2 and the addition of <br />mobile home deconstruction at the facility. <br />2. Observed operations of the CDLF. Currently working in Phase 2A. <br />3. The working face was small and well compacted. <br />4. No unacceptable items were observed in the working face. <br />5. The facility is currently stockpiling clean drywall, shingles and pallets to be recycled. An operations plan for the <br />items has been submitted and is under review. <br />6. Observed containers for metal and tires. The tires are hauled off once per week to US Tire Disposal in Cabarrus <br />County. <br />7. The monitoring wells were observed. Take measures to ensure that the monitoring wells are kept locked at all <br />times. <br />8. Take measures to ensure that all windblown litter is collected and returned to the working face at the end of each <br />operating day. <br />9. A small amount of erosion was observed on the slope of Phase 2. Continue to ensure that eroded areas are <br />repaired. <br />10. As required by 15A NCAC 13B .0542(1)(4), be sure leachate must be contained on -site or treated prior to <br />discharge. <br />11. Reviewed training records, groundwater and surface water sampling (June 2008 and October 2008), methane <br />monitoring, random inspection records, load refusal records, soil cover report and tonnage records. <br />12. This facility may receive solid waste that is generated within a service area consisting of Cabarrus, Mecklenburg, <br />Union, Stanly, and Rowan Counties in North Carolina, consistent with local government waste management <br />plan(s) and with local government approval. <br />13. The certified operators are as followed: <br />Don Sherrill - Manager of Landfill Operations Certification expires April 30, 2011. <br />April Carroll - Landfill Operations Specialist Certification expires October 8, 2011. <br />John Canup - Landfill Operations Specialist Certification expires February 9, 2010. <br />Fabian Howard — Landfill Operations Specialist Certification expires September 21, 2009. <br />Please contact me if you have any questions or concerns regarding this audit report. <br />Teresa N.
