Narrative: <br />Corporate Background and Experience <br />Project Staffing and Organization <br />Cost Estimate <br />Attachments: <br />General Contractor License <br />Business License <br />Certified Renovator Certification <br />Financial Statements <br />Insurance Certificates <br />Corporate Conflict of Interest Policy <br />Other Supporting Information (i.e., Resumes, Training Certificates, etc.) <br />ELECTRONIC FILING WAIVER <br />The Application MUST be submitted electronically to yheight(c)nccommerce.com However, <br />Applicants not able to obtain Internet access or for whom the electronic filing requirement causes <br />an undue hardship may request a waiver from the electronic filing requirement by writing to: <br />State Energy Office <br />N.C. Department of Commerce <br />ATTN: Electronic Submission Waiver Request <br />1340 Mail Service Center <br />Raleigh, N.C. 27699 -1340 <br />In the waiver request, the Applicant must clearly indicate why the electronic filing requirement <br />causes undue hardship and provide a list of all factors that make compliance difficult, expensive <br />or cumbersome. All waiver requests will be handled on a case -by -case basis. <br />REVIEW PROCESS <br />At 2:00 -p.m. on the closing date of each month (see Page 2), all emails and paper copies will be opened <br />and recorded. The review process will proceed in three stages: <br />1. Eligibility Review: Applications will be checked for completeness; incomplete responses will be <br />withheld from further review. <br />2. Recommendation by Committee: Complete responses will be sent to a review committee to be <br />scored per the evaluation criteria on page 17. The review committee will consist of at least three <br />members, comprised of SEO staff and non -SEO experts from the private sector, academia and <br />other governmental agencies who are qualified to determine the merits of the Application. The <br />Committee may recommend an Application for inclusion on the list of pre - qualified building shell <br />weatherization service providers or not recommend an Application. <br />3. SEO Review: The third stage review will be completed by Weatherization Assistance Program <br />staff who will determine which Applications shall be selected based on recommendations of the <br />review committee and feedback from the references provided. <br />APPROVED SUBCONTRACTORS LIST <br />No financial awards will be made from this Invitation to Submit Applications. The SEO will <br />endeavor to notify Applicants of rejection or of approval and resultant addition to the Approved <br />Subcontractors List within 90 -days of the SEC's receipt of their application. A letter will be sent via US <br />mail to the contact address listed on the Application Information Form.
