Cabarrus County <br />ADA Reasonable Accommodation Guidelines <br />The Department Head or Supervisor will notify the County's ADA Coordinator when they <br />become aware of a potential ADA reasonable accommodation request. <br />The ADA Coordinator will set up a meeting with the employee and discuss the ADA <br />related circumstances. <br />The ADA coordinator will provide the employee with an ADA Reasonable <br />Accommodation and ADA Medical Certification forms. <br />The ADA coordinator in coordination with Human Resources, Department Head and /or <br />Supervisor will engage in an interactive process with the employee. <br />The ADA coordinator will keep the Deputy County Manager(s) aware of any <br />circumstance(s) surrounding the ADA related request. <br />If necessary, legal representation may be contacted for further guidance. <br />Once the determination is made pertaining to the reasonable accommodation the <br />employee will be notified. <br />Note: Refer to ADA Title I — EEOC for guidance <br />Attachment number 6 <br />F -2 Page 71 <br />
