The contractor shall provide motor vehicle and equipment repair parts equal to or <br />exceeding the quality of replacement parts supplied by the original equipment <br />manufacturer. <br />Commercially purchased or contractor rebuilt parts may be used if they equal or exceed <br />the quality of the original parts. <br />CCTS may award multiple contracts for each maintenance activity. <br />For cost and service efficiencies, CCTS prefers to have a single Contractor that is able to perform <br />all services. However, if necessary, Contractors may subcontract specialty work to <br />subcontractors who have been approved prior to the start of work. The Contractor shall be <br />responsible for all costs associated with doing business with its subcontractors unless otherwise <br />mutually agreed upon with CCTS. <br />Service Facility Location and Contacts <br />A Contractor Information Sheet is included. Proposers must submit this form with their proposal. <br />When information on the form changes, a new form should be submitted. The form includes the <br />facility location and primary and secondary contact information. <br />PREVENTIVE MAINTENANCE SERVICE <br />The description of preventive maintenance activities follows: <br />Preventive maintenance is a term used to describe the performance of regularly scheduled <br />maintenance procedures of a vehicle to prevent the possibility of malfunctions. CCTS will <br />maintain all vehicles and wheel chair lifts in the best possible operational conditions. This will be <br />accomplished by adhering to and/or exceeding the manufacturer's recommended minimum <br />maintenance requirements. <br />Each CCTS transportation vehicle is assigned a number by the Transportation Supervisor which <br />is affixed to each vehicle in a visible location along with the phone number. <br />Every van driver is responsible for ensuring that periodic maintenance is performed on the <br />vehicle assigned to him/her at CCTS. The van driver will indicate on the Pre -Trip Inspection Form <br />when the vehicle is within 500 miles of the next scheduled service. <br />All requests for service must be forwarded to the Maintenance Supervisor. Repairs other than <br />scheduled maintenance must be submitted to the mechanic and repairs are posted on the <br />Maintenance Repair form. A copy of the form must be taken to the Service Center with the <br />vehicle and a copy of the form is filed with the Maintenance Supervisor and posted in the Vehicle <br />Maintenance Log. <br />In the event of a mechanical failure while the vehicle is in service, the driver will call the <br />Maintenance Supervisor to report the need for service.
