SCHEDULE D <br />THE HEALTH CARE PLAN OF CABARRUS COUNTY <br />SUMMARY OF BENEFIT AND CONTRIBUTION PROVISIONS <br />Benefits <br />~~ <br />Eligibility forAllotivance Employees who retire under the North Carolina Local <br />Governmental Employees Retirement System <br />(NCLGERS), and are actively employed with the County at <br />the time of retirement are eligible to participate in the <br />County's Retiree Health Insurance Plan until they reach age <br />65 or obtain Medicare, whichever comes first. <br />Amount ofA/loivairce The County will contribute to the cost of retiree insurance <br />premiums based on the years of service with Cabarrus <br />County using the following schedule for employees that <br />were hired before November 1, 2003: <br />Years of Service County <br />at Retirement Contribution <br />10 or more 100% <br />5 - 9 50% <br />OtJter Post Employment Benefits <br />The following schedule applies to those employees that <br />were hired on and after November 1, 2003: <br />Years of Service County <br />at Retirement Contribution <br />25 or more 100% <br />5 - 24 50% <br />The benefits will be paid to retirees at the same level as for <br />active members. <br />Health care and prescription drugs provided in the County's <br />retiree health care plan. Vision is included in the health <br />care plan. <br />Depe~rdeiit Covernge The retiree may continue dependent coverage (and pay the <br />full cost of this coverage) if enrolled in dependent coverage at <br />the time of retirement.
