Exhibit D <br />Miscellaneous: <br />1. Family and Medical Leave (FMLA) Records for Cabarrus County Employees will <br />be shared with the County Human Resource Department, in accordance with <br />applicable law. The medical records of Cabarrus County Employees treated for <br />Worker's Compensation injuries will be shared with the County Personnel <br />Director and Risk Management Department according to North Carolina <br />Industrial Commission regulations. Immunization records for work-related <br />Hepatitis B vaccinations and test of County Employees shall be shared with <br />County Human Resources and Risk Management. <br />2. Scope of Services to be provided: <br />a. Provider shall provide the following services: <br />i. Primary Care for all eligible clients <br />ii. Preventative health Care Education <br />iii. Routine Physicals <br />iv. First Aid <br />v. Treatment and management of work related Injuries, as appropriate <br />vi. Establishment of early return to work programs <br />vii. Pre-employment screens <br />viii. Drug and Alcohol testing <br />ix. Fitness for Duty Exams <br />x. Wellness Checks and standard Vaccinations <br />xi. Lab work <br />F-8 <br />
