Home and Community Care Block Grant for Older Adults <br />County Funding Plan <br />DOA-733 <br />Methodology to Address Service Needs of Low-income Minority Elderly and Rural Elderly <br />(Older American Act, Section 306 (a) (5) (A) (ii) <br />Community Service Providr Cabarrus County Department of Aging <br />County: Cabarrus <br />July 1, 2009 through June 30, 2010 <br />The Older Americans Act requires that the service provider attempt to provide services to low-income <br />minority individuals in accordance to their need for aging services. The community service provider <br />shall specify how the service needs of low-income minority elderly and rural elderly will be met through <br />the services identified on the Provider Services Summary (DOA-732). This narrative shalt address <br />outreach and service delivery methodologies that will ensure that this target population is adequately <br />served and conform to specific objectives established by the Area Agency on Aging for providing <br />services to low-income minority individuals. Additional pages may be used as necessary. <br />GOAL: <br />To ensure the participation in services provided through the Cabarrus County Department of Aging of the low <br />income minority older adults in the community. <br />The Cabarrus County Department of Aging provides services to all older adult residents regardless of race or <br />economic status. Brochures are distributed throughout the community that promote the services provided by our <br />agency. Services are also promoted through local churches and through speaking presentations to senior <br />citizens and local civic groups, <br />Our current client base includes a substantial number of minority clients and some of our LunchPlus Club sites <br />are located in minority and rural areas. Transportation is provided to all LunchPlus Club programs. We will <br />continue to distribute information in predominately minority and/or low income neighborhoods, promote our <br />services through local churches, make personal presentations to groups, and place informational brochures <br />in local medical offices/facilities. <br />We distribute our Sunset Outlook newsletter quarterly to 8,000 subscribers. <br />We also have started producing a twice weekly Television Show (Forever Young) that <br />addresses Aging issues and is available on local Cable TV. This allows publicity of our programs and <br />services to reach the entire county. We also coordinate service referrals with other human service <br />agencies and receive referals for services from various Iocai service providers. <br />Attachment number 1 <br />F-1 Page 88 of 627 <br />
