APPENDIX H Housing and Home Improvement <br />Service Provider Policies and Procedures for HCCBG <br />The Housing and Home Improvement service offered by Cabarrus County Department of Commerce will <br />promote independence and support aging-in-place and aging-in-community to the seniors of Cabarrus <br />County. This service will provide information and referral to those individuals wishing to remain in, return to <br />or locate safe affordable independent housing. It will identify health and safety issues affecting the home of <br />those individuals and provide needed home improvements or home modifications to enhance their mobility <br />and allow these citizens to remain within their community and avoid or postpone a move into a health care <br />setting. In order to pursue this mission, this agency agrees to: <br />1. Market services to and serve persons age 60 and above, who live within the county, and have no <br />one able or willing to perform services for them <br />2. Based on request for services, this agency will assess the need for service and target those with <br />greatest economic or social need <br />3. Maintain a waiting list and prioritize according to 10A NCAC 06E .0303, as needed <br />4. In addition to housing information and referral, provide any or all of the following services: <br />a) security enhancements (door knobs with reliable lock and key or dead lock bolts; windows fitted <br />with reliable locks; emergency response system installed; smoke and carbon monoxide <br />detectors) <br />b) minor home repairs (repair to primary bathroom sink, shower/tub or commode; repair to <br />kitchen sink; doorways widened; floors repaired; doors or windows repaired or replaced; <br />shingles and roofing felt replaced) <br />c) mobility and accessibility improvements (grab bar or handrail installation with solid backing; <br />thresholds modified; ramps built and installed within or adjacent to the home) <br />d) basic household furnishings and home appliance repair, replacement or purchase (chair; <br />mattress/box springs; stove; hot water heater; refrigerator; washing machine; heating or <br />cooling unit) <br />For the services specified above, this agency will collaborate with other agencies such as <br />Weatherization, Displacement Prevention Partnership-Vocational Rehabilitation, Urgent Repair, <br />Community Development Block Grant, US Department of Agriculture-Home Repair Loan and Grant <br />Program, etc. <br />Services requested not included above, will be requested as a Waiver Request, to the Area Agency <br />on Aging and Division of Aging and Adult Services. <br />5. This agency will not allow HCCBG funds to pay for rent, utility bills, food, medicine, security and <br />utility deposits, taxes. <br />6. This agency will not perform home improvements negatively affecting the structural integrity of a <br />home. <br />7. This agency will not provide home improvements which are an obligation of the landlord and will <br />have written approval prior to work being done to the property of a landlord. <br />8. This agency will not duplicate a home improvement service to the same housing unit for 3 <br />consecutive years following original service. <br />9. This agency will: <br />a) provide orientation, training or supervision for volunteers assisting with services <br />b) refer individuals to appropriate federal, state and local agencies for additional services <br />c) have the client receiving services sign the application and maintain records documenting those <br />financial and service activities <br />d) request reimbursement from DAAS for the actual project costs including administrative, labor <br />and materials - $1,500. or less per household per year <br />e) provide 2 opportunities for client to voluntarily contribute towards the cost of services received <br />without penalty for refusing <br />f) maintain confide iality of all record <br />~ Date: <br />Program Manager: <br />Attachment number 1 <br />F-1 Page 95 of 627 <br />
