Home and Community Care Block Grant for Older Adults <br />County Funding Pian <br />DOA•733 <br />Methodology to Address Service Needs of Low-income Minority Elderly and Rural Elderly <br />(Older American Aci, Section 306 (a) (6) (A) (ii} <br />Community Service Provider: Cabarrus County Department of Social Services <br />County: Cabarrus July 1, 2009 through June 30, 2010 <br />The Older Americans Act requires that the service provider attempt to provide services to low-income <br />minority individuals in accordance to their need for aging services. The community service provider <br />shall specify how the service needs of low-income minority elderly and rural elder{y will be met through <br />the services identified on the Provider Services Summary (DOA-732). This narrative shall address <br />outreach and service delivery methodologies that will ensure that this target population is adequately <br />served and conform to specific objectives established by the Area Agency on Aging for providing <br />services to low-income minority individuals. Additional pages maybe used as necessary. <br />GOAL: To increase participation in services and programs by low-income minority elderly persons at a <br />minimum rate of the percentage of low-income minority elderly in the community. <br />Cabarrus County Department of Social Services provides assistance to all individuals requesting <br />services regardless of race and with most of our programs serving low income individuals . The type or <br />extent of services clients receive are based solely on clients' need and availability of funding. <br />Through the HCCBG Program, 35% of the clients served are in the minority population. This <br />exceeds the 22.9% minority population in Cabarrus County, which is reflected in the 2006 census <br />estimated statistics. We will continue to strive to serve the low income minority population. In our <br />Community Alternatives Program (CAP), 37% of the Gients served are in this group. In addition, 80% <br />of the clients served through the In-Home Aide Program had income under 100% of the Federal <br />Poverty Level. <br />Outreach by the agency has been accomplished by newspaper articles, brochures, home visits by <br />social work staff, and contacts with other agencies. Since funding is limited for these programs, any <br />new outreach programs will not be implemented; however, current outreach efforts, which have <br />proven effective in the past, will continue. Based on current need for services, there are not <br />sufficient funds for an increase in the number of clients served. Requests from clients who meet the <br />need for In-Home Aide services will continue to be accepted. New clients will be added as existing <br />clients leave the program or experience a decrease in the number of units needed or additional <br />funds are secured. <br />As an agency with funding from several sources, assessments will be made with each case to be <br />sure that funding is used to best meet the needs of the clients and that Older American Act funds <br />are targeted towards low income minority individuals. <br />GOAL: To increase participation in services and programs by rural elderly in the community. <br />Cabarrus County Department of Social Servi~ provides assistance to all individuals living within <br />the county's boundaries requesting services regardless of location within the county. The type and <br />extent of services clients receive are based on clients' needs and availability of funding. <br />Through the HCCBG program, 5% of clients served are in more rural locations. Cabarrus County, <br />due to its location is described as part of "central counties of metropolitan areas of 1 million <br />population or more." It would be expected to find fewer rural residents in this county than in areas <br />that are not as populous. <br />Outreach by the agency hs been accomplished by newspaper articles, brochures, home visits by <br />social work staff, and contacts with other agencies. Since funding is limited for these programs, any <br />new outreach programs will not be implemented; however, current outreach efforts ,which have <br />proven effective in the past, will continue.
