Home and Community Care Block Grant for Older Adults <br />County Funding Plan <br />a0A-733 <br />Methodology to Address Service Needs of Low-income Minority Elderly and Rural Elderly <br />(Older American Act, Section 306 (a) (5) (A) (ii) <br />Community Service Provider: Cabarrus County Transportation Services <br />County; Cabarrus July 1, 2009 through June 30, 2010 <br />The Older Americans Act requires that the service provider attempt to provide services to low-income <br />minority individuals in accordance to their need for aging services. The community service provider <br />shall specify how the service needs of low-income minority elderly and rural elderly will be met through <br />the services identified on the Provider Services Summary (DOA-732). This narrative shall address <br />outreach and service delivery methodologies that will ensure that this target population is adequately <br />served and conform to specific objectives established by the Area Agency on Aging for providing <br />services to low-income minority individuals. Additional pages may be used as necessary. <br />GOAL: <br />The goal of Cabarrus County Public Transportation is to provide access to the public & clients, <br />County Agencies &Non-Profits <br />Service Delivery: <br />a) County is serviced county-wide <br />b) Service is interlined with Urban Concord/Kannapolis bus system to expand access <br />c) No clients are denied access for inability to pay <br />d) All clients are provided door to door services <br />e) All services are fully advertised on TV and print <br />f) 24 hour service line <br />Attachment number 2 <br />F-1 Page 114 of 627 <br />
