Cabarrus County Department of Aging <br />MEMORANDUM <br />To: Cabarrus County Board of Commissioners <br />From: Michael L. Murphy, CIRS-A MLA <br />Date: January 29, 2008 <br />Subject: Healthy Lives -Healthy Futures Funding <br />The attached Agenda Request form and Budget Amendment represents the Department of <br />Aging's request to accept the amount of $12,509 from the Cabarrus Health Alliance and the <br />Kate B. Reynolds Charitable Trust and to approve the new Wellness Coordinator position <br />within the pepartment of Aging. These funds are available as a result of a collaborative grant <br />award from the Kate B. Reynolds Charitable Trust to the Cabarrus Health Alliance and the <br />Department of Aging. The Department of Aging will hire a new Wellness Coordinator to <br />administer the Healthy Lives -Healthy Futures program for local order adults and adults with <br />disabilities. This program will target low income andlor minority residents of Cabarrus County <br />and will address chronic disease prevention and health maintenance. More specifically, this <br />program will target tobacco cessation, chronic disease education, health assessments. and/or <br />monitoring, nutrition education and increased physical activity for the target populations. This <br />new position and the resources available through the Healthy Lives •-Healthy Futures program <br />will allow the Department of Aging to expand our existing Wellness program and better serve <br />the older adults and adults with disabilities within our community. <br />The funding for this project will be available for a seven (7) year period. This is the second <br />grant that we have received from Kate B. Reynolds to expand the Wellness program through <br />the Department of Aging and Senior Center. The first five (5) year project was successful and <br />has been used as a model program by Kate B. Reynolds and across the state. <br />Thank you for your time and consideration.
