Contract N <br />(Contractor) <br />It is understood that the Contractor serves non-caretaker cases referred by law <br />enforcement and cases referred by other counties. It is understood the Contractor <br />diligently pursues reimbursement of costs for these services from other payers. it <br />is also understood it will not be the financial responsibility of Cabarrus County <br />Department of Social Services to reimburse Carolinas Medical Center-NorthEast <br />for the costs of multi-disciplinary assessments completed on children referred by <br />other sources than Cabarrus County Department of Social Services child welfare <br />staff. <br />• The Contractor agrees to maintain appropriate liability insurance as required by <br />law. <br />• The Contractor agrees to comply with the terms of the Americans with <br />Disabilities Act and all requirements imposed by or pursuant to the regulations <br />issued pursuant to that Section, which prohibit discrimination against handicapped <br />persons in employment and in the operation of programs and activities receiving <br />Federal Funds. <br />• Comply with Title VI and VII of the Civil Rights Act of 1964 and all <br />requirements imposed by or pursuant to the regulations issued pursuant to that <br />Title. <br />• This agreement may be amended or terminated upon mutual agreement of both <br />parties; or terminated by either party upon thirty (30) days prior notice in writing <br />to the other party, except that violation of State law or of established standards <br />concerning provision of the services(s) may result in termination of the agreement <br />without thirty (30) days prior notice. <br />The County: <br />• The County agrees to determine the clients' eligibility for any federal, state or <br />other funds which may help to meet the cost of the assessments performed under <br />this contract. <br />The usual course for referral will be the Child Medical Examination program <br />authorization form. <br />• The County will notify the contractor concerning the eligibility of each individual <br />for the.service, the period of time for which services are authorized, and any <br />changes in the individual's eligibility status. <br />• The County agrees to provide services as described in the CAC Operating <br />Procedures in support of multidisciplinary assessments. The provider agrees to <br />provide multidisciplinary assessment-related services in accordance with the CAC <br />Operating Procedures, <br />• The County may monitor the provider agency on-site quarterly but at least <br />annually and more frequently if the need arises to assure compliance with the <br />terms of the contract. The Vendor Agreement Provider Agencies Monitoring <br />Report for (DSS 6849) is used in the monitoring of the contract. <br />• The County will keep the contractor informed of all applicable Federal and State <br />laws, regulations, policies and standards governing the service program to which <br />the provider must adhere and of any alterations to these. <br />Contract-Scope of Work (06/04) F" ~ ~ Page 2of 4 <br />
