<br />Dear Ms. Barnhardt:
<br />As we discussed, I am enclosing duplicate, executed originals of a contract Amendment which will serve
<br />to formalize the July 1, 2007, renewal of our Agreement at the following compensation rates:
<br />Contract period: July 1, 2007, through June 30, 2008
<br />Base annualized fee: $484,495.76
<br />Base monthly fee: $38,707.98
<br />Per diem greater than 200: $1.70
<br />In addition, this correspondence serves to confirm Southern Health Partners' agreement to increase
<br />weekend and holiday nurse staffing hours at the Cabarrus County Jail from twelve hours per day to
<br />sixteen hours per day effective March 1, 2008, for an annual cost of $9,240.00. The County's annualized
<br />compensation rates will be adjusted accordingly effective March 1, 2008, to $473,735.76 ($39,477.98
<br />monthly.) To formalize the change in contract terms, I am enclosing duplicate, executed originals of a
<br />second Amendment which reflects the rate increase for additional staffing hours effective March 1, 2008,
<br />through June 30, 2008.
<br />Lastly, please hdte we a're requesting a-two percent (2%) increase in pricing for the contract period
<br />beginning July 1, 2008, as communicated to Chief Hunt by letter dated February 15, 2008. I am
<br />enclosing a copy of Mr. Reasons' letter to Chief Hunt for your file along with duplicate, executed originals
<br />of a third Amendment to confirm renewal and extension of our Health Services Agreement effective July
<br />1, 2008. Absent any modifications to the program during the contract period, the following compensation
<br />rates will be effective during the July 1, 2008, through June 30, 2009, contract period:
<br />Contract period: July 1, 2008, through June 30, 2009
<br />Base annualized fee: $483,210.48
<br />Base monthly fee: $40,267.54
<br />Per diem greater than 200: $1.70
