Home and Communhy Care Block Orant for Older Adults <br />County Funding Plan <br />OOA-733 <br />Methodology to Address Service Needsof Low-income Minority Eldedy and Rural Elderly <br />(Older American Act, Section 306 (a) (5) (A) (ii) <br />Community Service Provide Cabarrus County Department of Aging <br />County: Cabarrus July 1, 2008 through June 30, 2009 <br />The Older Americans Act requires that the service provider attempt to provide services to low-income <br />minority individuals in accordance to their need for aging services. The community service provider <br />shall specify how the service needs of low-Income minority elderly and rural elderly will be met through <br />fhe services IdentKed on the Provider Services Summary (DOA-732). This narrative shell address <br />outreach and service delivery methodologies that will ensure that this target population is adequately <br />served and wnform to specific obJedives established by the Area Agency on Aging for providing <br />servlces to low-income minority Individuals. Adtlltional pages may be used as necessary. <br />GOAL <br />To ensure the participation in services provided through the Cabarrus County Department of Aping of the low <br />income minority older adults In the community. <br />The Cabarrus County Depanment of Aging provides services to all alder adult residents regardless of rape or <br />economic status. Brochures ere distributed throughout the rnmmunity that promote the services provided by our <br />agency. Servlees are also promoted through local churches and through speaking presentations to senior <br />citizens and local civic groups, <br />Our current client base Includes a substantial number of minority clients and some of our LunchPlus Club sites <br />are located in minority and rural areas. Trensportatton Is provided to ell LunchPlus Club programs. We will <br />continue to distribute Information in predominately minority and/or low income neighborhoods, promote our <br />services through local churches, make personal presentations to groups, and place Informational brochures <br />in local medical of(ees/facilities. <br />We distribute our Sunset Outlook newsletter bi-monthly to 8,000 subscribers. <br />We also have started producing a twice weekly Television Show (Forever Young) that <br />addresses Aging issues and la available on local Cable TV. This allows publicity of our programs entl <br />services to reach the entire county. Weelso coordinate service referrals with other human service <br />agencies and receive referels for services from various local service providers. <br />r "~ <br />
