Northeast Psychiatric &Psychological Institute The County Agrees: • To determine the clients' eligibility for any Federal, State, or other funds which may help to meet the cost of the services performed under this contract. • To provide background information and on-going case updates. This shall include any information that will be beneficial to the provision of services with . the client. • To notify the contractor concerning the eligibility of each individual for the service, the period of time for which services are authorized, and any changes in the individual's eligibility status. This will occur through a written form, DSS- 5009. C. Rate per unit of Service (define the unit): 1. If Standard Fixed Rate, Maximum Allowable, (See Rates for Services Chart) 2. Negotiated County Rate. Cabarrus County Department of Social Services agrees to pay Northeast Psychiatric Services as follows: • Psychological Evaluation: $95.00 per hour • Psychological Testing: $95.00 per hour • Psychological Follow-Up/Consultation: $95.00 per hour • Substance Abuse Evaluation: $95.00 per hour • Intensive Out Patient: $95.00 per session • Individual Therapy: $95.00 per hour • Medication Eval for IOP: $200.00 • IOP Medication Follow-Up: $77.00 per visit • Psychiatric Evaluation: $200.00 • Psychiatric Medication Follow-Up: $58.00 per visit • Individual Group Assessment: $95.00 • On-Going Group Sessions: $50.00 per session The entire contract will not exceed $125,000.00 for the Fiscal Year 2007-2008. No fees will be charged to individuals determined to be eligible for services by the Cabarrus County Department of Social Services for the services provided under this contract unless otherwise directed to do so by Cabarrus County Department of Social Services.
