The employee <br />contribution toward the health insurance premium will be determined each year prior to <br />open enrollment. <br /> <br />Employees who falsify the tobacco use affidavit, will be subject to disciplinary action up <br />to and including dismissal in accordance with the Cabarrus County Personnel Ordinance. <br /> <br />HRA COMPLETION FOR ENROLLMENT <br /> <br />Specific Definition <br />A person who has successfully completed the Health Risk Appraisal on <br />www.mvCIGNA.col1l will be able to enroll in health insurance benefits, <br /> <br />Application <br />All employees and retirees who want to enroll in a Cabarrus County's health insurance <br />plan must complete the Health Risk Appraisal (HRA) on www.mvCIGNA.com. <br />Communication and training will be provided for all employees and retirees. <br /> <br />To enroll in health benefits beginning July I, 2006, employees and retirees must <br />complete the Health Risk Assessment (HRA) and an employee health screening, <br />Individual results are c<:>nfidential and will not be shared with the employer, The <br />employer will only have access to a general overview of group results for statistical <br />reporting, Compliance is tracked and reported prior to enrollment. <br /> <br />Employees and retirees who are not currently a CIGNA member will not have access to <br />complete a Health Risk Assessment. However, these employees and retirees will be <br />allowed to enroll in he~lth insurance benefit with the understanding that the Health Risk <br />Assessment must be completed by August 1,2006, New hires will complete the Health <br />Risk Assessment at the, next open enrollment period, <br /> <br />G--5 <br />
