<br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br /> <br />MEMORANDUM OF AGREEMENT <br />Between <br />Cabarrus County Governmental Center <br />And <br />Cabarrus Health Alliance/ <br />North Carolina Heart Disease and Stroke Prevention Program <br /> <br />This Memorandum of Agreement (MOA) is entered by and between the Cabarrus County <br />Governmental Center and the Cabarrus Health Alliance/ NC Heart Disease and Stroke <br />Prevention Program for the purpose of implementing the AED Pilot Program for the <br />South Central Region II of the State of North Carolina. This MOA is subject to the <br />provisions of all applicable Federal and State laws, regulations, policies and standards. <br />The site administrator for the Cabarrus County Governmental Center will be David <br />Montague, Safety Manager, Assistant Emergency Management Director, 65 Church <br />Street SE, Concord, NC 28026, 704-920-2102. The administrator for the Cabarrus <br />Health Alliance (CHA) NC Heart Disease and Stroke Prevention (HDSP) Program will <br />be Paige Waldrop, Regional HDSP Program Coordinator, 1307 S. Cannon Blvd, <br />Kannapolis, NC 28083, 704-920-1311. <br /> <br />This MOA may be terminated by either party upon at least 30 days written notice or <br />immediately upon notice for cause. <br /> <br />This MOA may be amended, if mutually agreed upon, to change scope and terms of the <br />MOA. Such changes shall be incorporated as a written amendment to this MOA. <br /> <br />Cabarrus County Governmental Center agrees to: <br />1. Develop, adopt, and carry-out policies and procedures for use and maintenance of <br />the AED; <br />2. Appoint and maintain a site coordinator for the AED pilot program; <br />3. Assign a Medical Director for the AED pilot program; <br />4. Coordinate and facilitate administration of public questionnaires before and after <br />placement of pilot AED; <br />5. Report to the HDSP coordinator any emergency use ofthe AED; <br />6. Assist with publicizing the site's participation in the NC AED pilot program and <br />enhance public awareness of heart disease, stroke and appropriate response to <br />medical emergencies; <br />7. Provide replies to the short questions that comprise periodic audit calls to the site; <br />8. Mount AED in a reasonably prominent location, and post approved signage of signs <br />and symptoms of heart attack, sudden cardiac arrest and call 9-1-1; <br />9. Use reasonable care in maintenance and security of the AED; <br />10. Assure that a sufficient number of volunteers/staff are CPRlAED trained (including <br />completion of pre/post training questionnaire) and that certification is maintained; <br />11. The following AED placement requirements: <br />· Place AEDs prominently on wall similar to mounting of a fire- extinguisher; <br />· Mount AED near a phone to help reduce the call-to-shock interval; <br /> <br />f--Cj <br />
