<br />NEMC Al!reements: <br />o Nurse Practitioner and Medical Assistant (25 hrs per week) <br />o Dr. Marc Kenneth Dy will serve as supervision physician for Family <br />Medicine. <br />o Dr. Steven St. Clair will serve as supervision for Occupational Medicine. <br />o Timeline: Hire a Nurse Practitioner 120 days after receiving bid <br />o NEMC will maintain all medical records in accordance with State and Federal <br />Guidelines, including HIP AA. <br />o Cabarrus County will be able to take advantage of the electronic <br />medical records within its Physician Network and Medical Center. <br />This allows continuity of care if the patient is seen at the Employee <br />Health Clinic or another NEMC physician Office and is admitted to <br />the hospital. <br />o NEMC will prepare data as requested and reports to Cabarrus County <br />regarding utilization, primary diagnosis, time spent at clinic and cost savings. <br />o NEMC's Pharmacy Service will coordinate the storage and dispensing of <br />select sample medication on site. <br />o Cabarrus County will benefit financially to purchase clinical supplies at a <br />reduced rate with NEMC. <br />o Illness prevention and risk reduction counseling and support groups <br />1. Smoking cessation <br />2. Weight management <br />3. Diabetes management <br />4. Injury prevention <br />Multiule Year Al!reement and Pricinl! <br />. NEMC is extremely excited and willing to commit to a three (3) year <br />agreement with Cabarrus County and its Employee Health Clinic. Due to <br />merit and CPI increases, NEMC will need to increase its charge slightly after <br />year one. <br />25 hour a week oution <br />. Year 1: Nurse Practitioner: $43,750 and Medical Assistant: $19,500 <br />plus 25% of employee benefits; Total: $79,062.50 <br />o Lab Tests will cost per test: Drug, strep, flu, urine dip, lipid <br />panel, Basic Metabolic Panel, urinalysis, immunization <br />Hepatitis A and B, Tetanus, Influenza and pneumonia. <br />. Year 2: An increase in the amount of 7.64% will be allocated to the <br />base salaries for the employees staffing the clinic ($85,102.88). <br />. Year 3: An increase not to exceed 8% will be allocated to the base <br />salaries of Year 2 for the employees staffing the clinic ($91,911.11). <br />40 hour a week oution <br />. Year 1: $126,500.00 <br />. Year 2: $136,164.60 <br />. Year 3: $147,057.76 <br /> <br />Exuenses Assumed bv NEMC <br />. Expenses NEMC was assuming and not passing on to Cabarrus County. The <br />following amounts are estimates provided by NEMC: <br />Medical Equipment: $ 5,000.00 <br />. Technology Expense (EMR): $10,000.00 <br />. Supervision by MD: $10,000.00 <br />Administrative Supervision: $ 5.000.00 <br />Total Estimated Expense: $30,000.00 <br />F22. <br />
