This entails family intervention and coordination of services. The psychological evaluation is a clinical <br />interview in which the staff psychological associate considers emotional, developmental, social and environmental <br />issues that may contribute to the presenting problem. Where therapeutically indicated psychological <br />assessment/testing may be provided. Based on these findings, treatment recommendations and referrals will be <br />made. Actual delivery of treatment services will be outside the scope of this grant; however, DA YMARK will be <br />available for services including: individual, family and group therapy; anger management, substance abuse treatment, <br />intensive famil reservation se 'ices, s chiatric consultations and medication assessment. <br />COMPONENT NARRATIVE (attach for each component) <br />~ll!mpP!!!lJflt Narrative .(attacforeach component) <br />(1) Describe the physical facilities, including the address, in which the services will be delivered. If services are <br />delivered at multiple locations, plf;Jase include information for all locations, <br />Cabarrus County Court House, union Street, Concord. NC is the site for these services for the primary grant position <br />held by the Staff Psychological A~sociate-Human Services Clinician. DAYMARK is the site of all other service delivery <br />if the client and family is referred 'for and chosses additional services not covered by the grant. <br /> <br /> <br />(2) Identify paid or volunteer sta qualifications, (certifications, degrees, work experience) and their responsibilities <br />relative to this com onent. <br />Clinical Site Director supervises II aspects of the clincial services. Mental Health Team Leader provides day-to-day <br />supervision to all staff associatet\.with the DJJDP funded programs. JCPC Psychological Associate-Human Services <br />Clinlcian-Master's level clinician, provides on-site psychological services detivery to DJJDP clients and their families, <br />and coordinates referrals and cOl'lsultations to the court. DA YMARK staff providing services to referred clients hold a <br />minimum of a Master's degree in related mental healthihuman service field and hold a national certification andlor <br />professional licensure in their respective discipline or in the process of achieving such status.
