<br />L:r~_~Ut~c~()\V le~~_~sth~t)l1_re_c_eivillg,s_t()ring,..processin~ or ()thef\Vi_se l1sill~ or _ .... . <br />dealing with any such information, the Provider and its employees are fully bound by <br />the provisions of North Carolina General Statutes 122-8.1 as amended and all other <br />Federal and State Laws and Regulations that govern and guarantee the treatment <br />rights and confidentiality of individuals receiving substance abuse treatment services. <br /> <br />B, To resist any effort to obtain access to client treatment information not expressly <br />provided for in the aforementioned paragraph. <br /> <br />C. Provider shall obtain appropriately signed forms allowing for the release of <br />information that may be necessary for the Provider to conform to the requirements of <br />the NC State Criminal Justice Partnership Program. <br /> <br />SECTION XII: EQUIPMENT COSTS AND OFFICE SPACE <br /> <br />The Provider shall supply, at its own expense, all equipment, tools, materials and/or <br />supplies required to deliver treatment services contracted hereunder, The Provider shall <br />provide office space for necessary staff members. <br /> <br />SECTION XIII: CLIENT ADMISSION <br /> <br />In accordance with the provisions of Title N of the Civil Rights Act of 1963, and the <br />regulations of the United States Department of Health and Human Services, issued <br />pursuant thereto, the Provider agrees to extend all funds allocated in such a manner that <br />no person will be excluded from participation in, be denied the benefits of, or otherwise <br />be subj ect to discrimination under purchased services on the grounds ofrace, religion, <br />color, nation origin or sex. <br /> <br />SECTION XIV: ENTIRE AGREEMENT <br /> <br />This Contract, including the attachments referenced herein being the original proposal <br />from CARE dated 04/30/2001, and any amendments that are annexed hereto, represent <br />the entire agreement between the County and the Provider and supersedes all prior verbal <br />and written statements or agreements. In the event of conflict between the terms and <br />conditions of this agreement and the above referenced documents, the terms and <br />conditions of this agreement and its amendments shall govern, This Contract may only <br />be amended by written amendments duly written and executed by the County and the <br />Provider. <br /> <br />8 <br /> <br />F- 1.. <br />
