<br />3 <br /> <br />(d) Application for a certificate to function as an individual first responder in the county by an <br />applicant shall be filed with the Ceounty Ddepartment of Eemergency MlfIedical Sservices by <br />the applicant, fire chief or hislher designee with proper approval from the Ceorporate Booard of <br />Ddirectors. This application shall be made upon such forms prepared by the Ceounty <br />Ddepartment of Eemergency MlfIedical Sservices and shall contain: <br /> <br />(I) The name, age, social security number, driver's license number, and address of the applicant <br />individual. <br /> <br />(2) State MlfIedical Rfesponder expiration date or Sstate Eemergency MlfIedical Ttechnician <br />expiration date. <br /> <br />(3) The name of the first responder agency with which the applicant individual is to be affiliated. <br /> <br />( e) Upon receipt of application for certificate to operate as an individual first responder in the <br />county, the Ddirector of Eemergency MlfIedical Sservices or his designated representative will <br />approve or disapprove all applications based on criteria in sec.tion 26-55. <br /> <br />(Ord. of 12-7-92(2), ~ 3) <br /> <br />Sec. 26-54. Terms of certification as a first responder agency and individual first <br />responder. <br /> <br />(a) A certificate to operate as a first responder agency in the county and issued by the Ceounty <br />Ddepartment of Eemergency MlfIedical Sservices to an applicant agency will be permanent, with <br />a minimum of four individual first responders. However, either party at its option may terminate <br />the certificate upon 30 days prior written notice to the other party or immediately upon failure to <br />meet qualification standards. <br /> <br />(b) If any first responder agency or individual first responder shall violate or fail to comply with <br />any provision of these guidelines, notification will be given by the Ddirector of the Ceounty <br />Eemergency MlfIedical Sservices to the first responder agency fire chief or hislher designee <br />stating the violation and requesting a meeting to resolve the violation. If the violation is not <br />resolved, the written complaint will be forwarded to the Ceounty FfIrst Rresponder Aadvisory <br />Beoard for its investigation of the complaint. The Ffust Rfesponder Aadvisory Booard will have <br />the fmal authority on suspension or revocation of an individual first responder's certification. The <br />Bboard can make a recommendation to the Booard of C60unty Ceommissioners for suspension <br />or revocation of a first responder agency's certificate. <br /> <br />(c) Upon termination, revocation, or suspension of a certificate to operate as a first responder <br />agency or individual, as determined under this article, the first responder agency or individual <br />shall immediately cease to operate. Termination of a certification for noncompliance to rules <br />must be approved by the Cabarrus County Booard of 6ellfity Ceommissioners. <br /> <br />(d) If membership drops below that specified in subsection (a) of this section the first responder <br />unit has 120 days to get back to strength. If not, the Aadvisory Bboard will suspend the <br />certification until the number is back to minimum strength. <br /> <br />r~15 <br />
