Murphy, CIRS-A v."~ <br />May 6, 2005 <br /> <br />From: <br /> <br />Date: <br /> <br />Subject: <br /> <br />Home and Community Care Block Grant <br />Lead Agency Designation <br /> <br />Enclosed please find the Designation of Lead Agency for the Home and Community Care Block <br />Grant (HCCBG) form for Cabarrus County. We respectfully request that the Cabarrus County <br />Department of Aging be reappointed to serve as the lead agency for the HCCBG for Cabarrus <br />County. This designation no longer requires annual appointment by the Board of <br />Commissioners. Therefore, we are requesting that this designation be made effective until <br />further notice. The primary responsibilities of the HCCBG Lead Agency are to convene and <br />facilitate the HCCBG Steering Committee in an effort to develop a funding allocation proposal <br />for consideration by the Board of Commissioners. We also work with all of the agencies that <br />receive HCCBG funding to develop the annual funding plan for Cabarrus County. As lead <br />agency, the Department of Aging serves as the liaison to the Centralina Area Agency on Aging <br />and the North Carolina Division of Aging and Adult Services. <br /> <br />Thank you for your time and consideration.
