<br />DOA-733 <br /> <br />Home and Community Care Block Grant for Older Adults <br />County Funding Plan <br /> <br />Methodology to Address Service Needs of Low-income Minority Elderly and Rural Elderly <br />(Older American Act, Section 306 (a) (5) (A) (ii) <br /> <br />Community Service Provider: <br /> <br />Cabarrus County Public Transportation <br /> <br />County: Cabarrus County <br /> <br />July 1, 2005 through June 30, 2006 <br /> <br />The Older Americans Act requires that the service provider attempt to provide services to low-income <br />minority indMduals in accordance to their need for aging services. The community service provider <br />shall specify how the service needs of low-income minority elderly and rural elderly will be met through <br />the services identified on the Provider Services Summary (DOA-732). This narrative shall address <br />outreach and service delivery methodologies that will ensure that this larget population is adequately <br />served and conform to specific objectives eslablished by the Area Agency on Aging for providing <br />services to low-income minority individuals. Additional pages may be used as necessary. <br /> <br />GOAL: <br /> <br />The goal of Cabarrus Count Public Transportation is to provide access to the public and clients <br />County Agencies and Non- Profits <br /> <br />Service Delivery: <br />a) County is serviced county wide <br />b) Service is interlined with Urban ConcordlKannapolis bus system to expand access <br />c) No client are denied access for inablity to pay <br />d) All clients are provided door to door services <br />e) All services are fully advertised on 1V and print <br />f) 24 hour sewrvice line <br /> <br />1=". ~ i.f <br />
