These objectives must include participan/outcomes and may inciude program outcomes. <br /> <br /> <br />1) Provide in-home counseling and support to families in order to reduce out-of-home placement of juveniles <br />who are at risk of system placement.-- 90% of cases served will have children remaining in the home following <br />Home Based intervention. <br />(2) 85% of juveniles served will demonstrate improved behaviors at home (such as less aggression; abiding by <br />household rules and curfew; deduction in runaway behavior, better communication and conflict resolution skills). <br />(3) 80% of juveniles served wili demonstrate improved behaviors at school (less aggression, truancy, improved <br />academic performance). . <br />(4) 85% of families served will demonstrate Improvement in the childlparent relationship following Home Based <br />intervention, with parents demonstrating enhanced techniques in parenting style. <br />(5) Reduction in further court referrais one year following case closure--90%. . <br />(6) 95% of cases will receive follow-up services at quarterly intervals. <br />(7) 60% of cases will have 16-24 face-to-face visits before completion of home based intervention. <br />(8) 100% of cases will have detailed case plans to outline objectives and goals, which will be assessed and <br />evaluated for changes monthly. <br />(9) 25% of children served will be referred to mentoring services to further impaCting success of objectives. <br />(10) 100% of cases will be evaluated for supplemental parenting education classes to enhance objectives <br />taught to parents during Home Based Services. <br /> <br />State how riorit risk factor s identified b the JCPC will be addressed. <br />Families will have help through Home Based intervention in addressing and appropriately responding to issues <br />that are prevalent in impoverished neighborhoods and in today's society as a whole, such as negative peer <br />relations or gang involvement, crime and violence, drug and alcohol use, apathy in the schools, and aggression. <br />Family conflict and management issues will be evaluated and addressed through frequent home visits to <br />determine barriers to change. These issues may involve substance abuse, domestic vioience, or abuse and <br />neglect issues. Available resources will be utilized to help effect change affecting safety issues. Family <br />communication will be encouraged by developing strategies to effectively discuss issues with less confrontation. <br />Resources will also be sought to facilitate the adolescent's need for support, such as Big Brothers Big Sisters <br />(BBBS). Through parenting education, parents will be shown the importance of becoming more involved with <br />their children and to provide supervision and structure In order to appropriately enforce bQundaries. <br />Encouraging the adolescent to take responsibility for behavior and change facilitates the process to meet the <br />challenges in the adolescent's world. Prot!3ctive factors promote positive behavior, health, w!3l1-being and personal success. This is the core <br />of what this program is design!3d to do. Home Based Services is highly utilized to help clarify and assimilate <br />prot!3ctiv!3 factors to counteract risk factors. The Home Based program focuses. on heiping families improve their <br />skills in these areas: communication, problem-solving, anger management, identifying strengths and positives, <br />improving coping skills, accessing appropriate resources in the community, l!3arning how to negotiate, s!3tting <br />goals and working towards achieving them, and enhancing family relationships. The Home Based Counselors <br />have an opportunity to contribute to early identification of substance abuse problems because they are regularly <br />visiting the family's home. Home Based Services processes with the family the importance of building better <br />communication between the parent and child, as well as providing parenting education to strengthen parental <br />disciplinary approaches, such as being consistent and setting forth clear boundaries, standards,and <br />expectations. While observing interactions in the home, and witnessing conversation ai"id responses to various <br />situations, Home Based Services has the opportunity to redirect and coach the parent on suitable responses <br />and actions to help counteract issues.
