<br />3. Lutheran Services will schedule a physical exam for each <br />resident within 7 days of admission. <br /> <br />4. Lutheran Services will meet with the school at each child's <br />enrollment to discuss his educational plan and will review this plan with the <br />school each month. <br /> <br />5. The County shall pay to Luthyran Services the sum of <br />$17,445.11 per month for July 2005, through June 2006, to maintain said <br />horne, to provide food, shelter, and services herein contemplated. The County <br />will provide clothing as determined by DSS based on the individual needs of <br />the child. <br /> <br />6. The County through DSS will sign the admission <br />agreement at the time of placement of a child. <br /> <br />7. The County through DSS requires that no equipment, <br />furnishings or other items be purchased out of service fees (JCPC and 90unty <br />funds) without prior approval ofDSS. <br /> <br />8. Lutheran Family Services will keep a no smoking policy in <br />effect for the minors who are group home residents. <br /> <br />9. The County shall be responsible for the payment of any <br />medical expenses incurred by any child placed in the home other than routine <br />first aid administered by the home; provided, Lutheran Services may authorize <br />emergency medical treatment by licensed physicians should circumstances <br />require. Reimbursement for medical services is limited to Medicaid coverage <br />only. <br /> <br />10. The County through DSS shall have the right to inspect the <br />premises of the home at any reasonable hour in order to ascertain that the <br />children placed in the group home are receiving adequate care; and Lutheran <br />Services agrees to secure and maintain all licenses nece,ssary to operate the <br />facility and to notify the agency immediately upon the loss of any required <br />license or provisional license status. <br /> <br />11. As hereinabove set forth, the beds and services provided in <br />the home shall be for the exclusive use and benefit of children in the custody <br />of the DSS and Lutheran Services shall not permit such facility to be occupied <br />by children from any other agency without the express written agreement of <br />DSS. <br /> <br />1='-5 <br />
