If the report is received after this date, a penalty for <br />failure to file return and a penalty for failure to pay tax when due is applicable. The <br />reports and payments were not postmarked until March 16, 2004. The penalty for failure <br />to file return is $314.50. The penalty for failure to pay tax when due is $629.00 <br /> <br />Please remit $943.50 in penalties. Please make every effort to file in a timely manner in <br />the future.
