Home and Community Care Block Grant for Older Adults <br /> County Funding Plan <br /> <br />DOA-733 <br /> <br />Methodology to Address Service Needs of Low-~ncome M~nonty Elderly and Rural Elderly <br /> (Older American Act, Section 306 (a) (5) (A) <br /> <br />Commumty Service Prowdel Cabarrus County Department of Aglnc, I <br /> <br />County' Cabarrus July 1, 2004 through June 30, 2005 <br /> <br />The Older Amencans Act requires that the service provider attempt to prowde services to iow-income <br />minority individuals in accordance to their need for aging services. The community service provider <br />shall specify how the service needs of Iow-income minority elderly and rural elderly w~ll be met through <br />the services identified on the Provider Services Summary (DOA-732). This narrative shall address <br />outreach and service delivery methodologies that will ensure that this target population is adequately <br />served and conform to specific objectives established by the Area Agency on Aging for providing <br />services to Iow-~ncome minority individuals. Additional pages may be used ~s necessary <br /> <br />GOAL: <br /> <br />To ensure the participation In services provided through the Cabarrus County Department of Aging of the Iow <br />~ncome m~nonty older adults in the community <br /> <br />The Cabarrus County Department of Aging prowdes services to all older adult residents regardless of race or <br />economic status. Brochures are distributed throughout the commumty that promote the services prowded by our <br />agency Services are also promoted through local churches and through speaking presentabons to senior <br />cRizens and local civic groups, <br /> <br />Our current client base ~ncludes a substantial number of m~nonty clients and some of our LunchPlus Club s~tes <br />are located in minority and rural areas. Transportation ~s provided to all LunchPlus Club programs We w~ll <br />conhnue to distnbute information ~n predommately minority and/or Iow income neighborhoods, promote our <br />services through local churches, make personal presentabons to groups, and place ~nformabonal brochures <br />in local medical offices/faclhbes <br /> <br />We d~str~bute our Sunset Outlook newsletter b~-monthly to 10-12,000 subscnbers. <br />A second newsletter (Semor Sawy) Is distnbuted monthly This publicahon ~s distributed <br />on counter tops throughout the rural areas of Cabarrus county. We also have started producing a twice weekly <br />Television Show (Forever Young) that addresses Aging issues and is available on local Cable TV. <br />This allows pubhc~ty of our programs and services to reach the enbre county We also coordinate service <br />referrals with other human service agencies and receive referrals for services from various local service <br />providers <br /> <br /> <br />
