Home and Community Care Block Grant for Older Adults <br /> County Funding Plan <br /> <br />DOA-733 <br /> <br />Methodology to Address Service Needs of Low-~ncome Minority Elderly and Rural Elderly <br /> (Older American Act, Section 306 (a) (5) (A) (ii) <br /> <br />Community Service Provider <br /> <br />County: Cabarrus <br /> <br />Cabarrus Transportation Department <br /> <br />July 1, 2004 through June 30, 2005 <br /> <br />The Older Americans Act requires that the service provider attempt to provide services to Iow-income <br />minority individuals in accordance to their need for aging services. The community service provider <br />shall.specify how the service needs of Iow-income minority elderly and rural elderly will be met through <br />the services identified on the Provider Services Summary (DOA-732). This narrative shall address <br />outreach and service delivery methodologies that will ensure that this target population ~s adequately <br />served and conform to specific objectives established by the Area Agency on Aging for providing <br />services to Iow-income minority individuals. Additional pages may be used as necessary. <br /> <br />GOAL: <br /> <br />The goal of Cabarrus County Transportabon ~s to prowde access to the public and chents of County Agencies and Non-Profits <br /> <br />Service Delivery: <br />a. County is serviced county wide <br />b. Service is interlined with the new Urban Concord/Kannapolis bus system to expand access <br />c. No clients are denied for inability to pay. <br />d.AII accessed clients are provided door to door services. <br />e.AII services are fully advertised on TV and print. <br />f. 24 hour service line. <br /> <br /> <br />
