Home and CommUnity Care Block Grant for Older Adults <br /> -.County Funding Plan <br /> <br />DOA-733 <br /> <br />Methodology to Address Service Nee~Js of Low-income Minority Elderly and Rural Elderly <br /> (Older American A~t, Section 306 (a) (5) (A) (ii) <br /> <br />Community Service Prowde~ Cabarr~s Count~/Planmn~ Department <br /> <br />County' Cabarrus ~ July 1, 2003 through June 30, 2004 <br /> <br />The Older Americans Act requires th* the service provider attempt to provide services to Iow-income <br />minority individuals in accordance to ~eir need for aging services. The community service provider <br />shall specify how the service needs o{'iow-income m~norlty elderly and rural elderly will be met through <br />the services identified on the Provid~ Services Summary (DOA-732) This narrative shall address <br />outreach and service dehvery metho~31ogies that will ensure that this target population is adequately <br />served and conform to specific objeclJves established by the Area Agency on Aging for providing <br />services to Iow-income minonty indiv~uals. Additional pages may be used as necessary <br />GOAL <br /> <br />The Planning Services Dept goal is ~ coordinate and provide Transportation Services and <br />Housing and Home Improvement Se~/ices with the Dept of Aging and funded by the HCCCBG. <br />Coordination of these functions will pl~duce a service product that is both accountable and accessible <br />by the full scope of clients for which t~e HCCCBG funds were designated. <br />It is the intent to meet the needs of t~t specialized population <br />Planning Services will coordinate witll the Department of Aging, DSS', and other service providers <br />to insure that the target population i~served All eligible clients will be provided access to <br />service. Planning Services will follov~all program requirements for service provision and client <br />treatment Transportation will be pro{ided for authorized purposes to eligible clients Homes will <br />be ~mproved or rehabilitated for eligil~e clients. Improvements include, but are not limited to, <br />handicap accessibility, bathroom ~mp~ovements handicap accessible or otherwise, roof repairs, <br />and safety improvements. All improvements w~ll be made ~n compliance w~th all program regulations. <br />Th Planning Services Department wil~take referrals from the Department of Aging, DSS, and other <br />service prowders to the target popull~ion. Outreach is achieved through these programs, word of <br />mouth and visiting nutrition sites and ~ther special programs designed to address the target <br />population. : <br /> <br /> <br />
