Act, Section 306 (al (5) (^) (ii) <br /> <br />Community Service Provide~ Ca?,rrus County Department of A~in~l <br /> <br />County: Cabarrus i July 1,2003 through June 30, 2004 <br /> <br />The Older Americans Act require~ that the service provider attempt to provide services to iow-income <br />minority individuals in accordance .~ their need for aging se~ces. The community service provider <br />shall specify how the service nee¢ ~ of iow-income minority elderly and rural elderly will be met through <br />the se~ces identified on the Prov der Services Summary (DOA-732). This narrative shall address <br />outreach and se~ce delivery met ~0dologies that w~ll ensure that this target population is adequately <br />served and conform to specific ob ec~es estab§shed by the Area Agency on Aging for providing <br />services to Iow-income minonty in~ Ilviduals. Additional pages may be used as necessary. <br /> <br />GOAL: ~ , <br /> <br />To ensure the participation in services provided through the Cabarrus County Department of Aging of the Iow <br />income minority older adults in the icommunity. ~ <br /> <br />The Cabarrus County Departmenli of Aging provides services to all older adult residents regardless of race or <br />economic status. Brochures are ¢ sl~ibuted throughout the community that promote the services provided by our <br />agency. Services are also promol ~d through local churches and through speaking presentations to senior <br />citizens and local civic groups, <br /> <br />Our current client base includes a ,~bstantial number of minority clients and some of our LunchPlus Club sites <br />are located in minority and rural al =as. Transportation is provided to all LunchPlus Club programs. We will <br />continue to distribute information il predominately minority and/or Iow income neighborhoods, promote our <br />se~ces through local churches, r~ ake personal presentations to groups, and place informational brochures <br />in local medical offices/facilities. ~ <br /> <br />~ewsletter bi-monthly to 10-12,000 subscribers. <br />t~) is distributed monthly.
