Home and Community Care Block Grant for Older Adults <br /> <br />DOA-733 <br />(Rev. 1/01) <br /> <br /> County Funding Plan <br /> <br /> Methodology to ,Address Service Needs of Low-Income Minority Elderly <br /> (Older Americans Act, Section 306(a)(5)(A)(ii)) <br /> <br />Community service Provider i Cabarrus County Department of Social Services <br /> <br />County Cabarrus , July 1, 2003 through June 30, 2004 <br /> <br />The Older Americans Act re~luired that the service provider attempt to provide services to low- <br />income minority individuals ~ accordance to their need for aging services. The community <br />service provider shall specif~, how the service needs of low-income minority elderly and rural <br />elderly will be met through t~e services identified on the Provider Services Summary (DOA-732). <br />The narrative shall address o~.treach and service delivery methodologies that will ensure that this <br />target population is adequately served and conform with specific objectives established by the <br />Area Agency on Aging, for providing services to low-income minority individuals. Additional <br />pages may be used as necessary. <br /> <br />GOAL: To increase par~cipation in services and programs by low-income minority elderly <br />persons at a minimum rate of,~he percentage of low-income minority elderly in the community. <br /> <br /> Cabarrus County Department of Social Services provides assistance to all individuals <br />requesting services regardles~ of race with most of our programs serving low income individuals. <br />The type or extent of servic~ clients receive are based solely on clients' need and availability of <br />funding. <br /> <br /> Through the HCCBC~,,Program, 23% of the clients served are in the minority population. <br />This exceeds the 15.7% min4rity population of Cabarrus County, which is reflected in the 2000 <br />census statistics. We will continue to strive to serve the low-income minority population. In our <br />Community Alternatives Pro,am (CAP), 35% of the clients seated are in this group. In addition, <br />52% of the clients served through the In-Home Aide Program had income under 100% of the <br />Federal Poverty Level. <br /> <br /> Outreach by the age~y has been accomplished by newspaper articles, brochures, home <br />visits by social work staff, aid contacts with other agencies. Since funding is limited for these <br />programs, any new outreacl~ programs will not implement; however, current outreach efforts, <br />which have proven effective ~n the past, will continue. Based on current need for services, there <br />are not sufficient funds for a !significant increase in the number of clients served. Requests from <br />clients who meet the need foi In-Home Aide Services will continue to be accepted. New clients <br />will be added as existing clients leave the program or experience a decrease in the number of units <br />needed or additional funding ~ secured. <br /> <br /> As an agency with fur~. ing from several sources, assessments will be made with each case <br />to be sure that funding is use~_ to best meet the needs of the clients and that Older Americans Act <br />funds are targeted towards loW-income minority individuals. <br /> <br /> <br />
