Murphy <br /> <br />Date: October 5, 2003 <br /> <br />Subject: <br /> <br />Revised Funding Plan FY'04 - Home and Community Care Block Grant <br /> (Increased funding of $5,247) <br />The 30 Minutes for Life Grant- Grant funding by the Robert Wood Johnson <br /> Foundation totaling $23,920 <br /> <br />Please review and approve the attached revised Cabarms County Funding Plan for the Home and <br />Community Care Block Grant (HCCBG) for FY'04. We have also included Budget Amendment <br />Request form for your approval that will allow us to receive and spend these funds. The increase <br />that we are receiving is based on the final state budget that was approved by the General <br />Assembly. Our current HCCBG Funding Plan is based on estimated figures that we received <br />from the NC Division of Aging last spring. Also included in this requested budget amendment is <br />an increase in funding that is possible due to the recent award of a grant from the University of <br />Illinois. This grant is funded by the Robert Wood Johnson Foundation and will provide funding <br />for our Senior Center Wellness program to develop a program called 30 Minutes for Life. This <br />program will target high-risk older adults and utilize community resources to enhance the quality <br />of life for our local seniors. <br /> <br />This budget amendment request does not require any additional local funding as a match. We <br />are able to meet the requirements for matching funds by utilizing existing services as an in-kind <br />contribution to these programs. <br /> <br />Thank you for your time and consideration.
