State of North Carolina County of Wake Weatherization Assistance Program (CFDA #81.042) and Heating and Air Repair and Replacement Program (CFDA #93.568) Amendment # 1 Contract # 450 This agreement amends the contract beating the effective date of July 1, 2003 between the North Carolina Department of Health and Human Services, Office of Economic Opportunity, hereinafter referred to as the "Office", and the Cabarms County Planning Department, hereinafter referred to as the "Contractor". As provided for under the terms of this contract, the Office and the Contractor agree to amend the following provisions: 12. Entire Agreement: This contract and any documents incorporated specifically by reference represent the entire agreement between the parties and supersede all prior oral or written statements or agreements. This contract, Attachment 1, Title 10 Code of Federal Regulations Part 440, Rule NCAC Title 10 Chapter 51, the contractor's application and any addenda thereto are incorporated herein by reference as though set forth verbatim. AND 16. Responsibilities of the Contractor: "Responsibilities of the Contractor": Attachment I, Work Program and Budget, details the activities that will be performed in accordance with the terms of this amended contract. AND 18. Amount of Reimbursement: The not-to-exceed amount is being increased from Seventy-Three Thousand Twenty Seven ($73,027) Dollars to Ninety-Nine Thousand Five Hundred Fifty-Eight ($99,558) Dollars Weatherizataon Assistance Program; CFDA # 81.042 and from Eighteen Thousand Five Hundred Ninety-Six ($18,596) Dollars to Twenty-Seven Thousand Nine Hundred Twenty- Seven ($27,927) Dollars Heating and Air Repair and Replacement Program; CFDA # 93.568. There are no matching requirements from the contractor.
