State of North Carolina <br />County of Wake <br /> <br /> Weatherization Assistance Program <br /> (CFDA #81.042) <br /> and <br />Heating and Air Repair and Replacement Program <br />(CFDA #93.568) <br /> <br />Amendment # 1 <br /> <br />Contract # 450 <br /> <br />This agreement amends the contract beating the effective date of July 1, 2003 between the North <br />Carolina Department of Health and Human Services, Office of Economic Opportunity, <br />hereinafter referred to as the "Office", and the Cabarms County Planning Department, <br />hereinafter referred to as the "Contractor". <br /> <br />As provided for under the terms of this contract, the Office and the Contractor agree to amend <br />the following provisions: <br /> <br />12. Entire Agreement: <br /> <br />This contract and any documents incorporated specifically by reference represent the entire <br />agreement between the parties and supersede all prior oral or written statements or <br />agreements. This contract, Attachment 1, Title 10 Code of Federal Regulations Part 440, <br />Rule NCAC Title 10 Chapter 51, the contractor's application and any addenda thereto are <br />incorporated herein by reference as though set forth verbatim. <br /> <br />AND <br /> <br />16. Responsibilities of the Contractor: <br /> <br />"Responsibilities of the Contractor": Attachment I, Work Program and Budget, details the <br />activities that will be performed in accordance with the terms of this amended contract. <br /> <br />AND <br /> <br />18. Amount of Reimbursement: <br /> <br />The not-to-exceed amount is being increased from Seventy-Three Thousand Twenty Seven <br />($73,027) Dollars to Ninety-Nine Thousand Five Hundred Fifty-Eight ($99,558) Dollars <br />Weatherizataon Assistance Program; CFDA # 81.042 and from Eighteen Thousand Five <br />Hundred Ninety-Six ($18,596) Dollars to Twenty-Seven Thousand Nine Hundred Twenty- <br />Seven ($27,927) Dollars Heating and Air Repair and Replacement Program; CFDA # <br />93.568. <br /> <br />There are no matching requirements from the contractor. <br /> <br /> <br />
