At a minimum, these <br />contacts are attempted 6 months after case closure and 1 year after case closure. Follow-up contacts are <br />listed on the case log. In addition to this, the Home Based workers and/or supervisor will contact Juvenile <br />Services Intake one year after case closure to see if the client had additional court involvement. <br /> <br /> <br />
