DOA-733 <br />(Rev. 1/01) <br /> <br />Home and Community Care Block Grant for Older Adults <br /> <br />County Funding Plan <br /> <br />Methodology to Address Service Needs of Low-Income Minority Elderly <br /> (Older Americans Act, Section 306(a)(5)(A)(ii)) <br /> <br />Community service Provider Cabarrus County Department of Social Services <br /> <br />County Cabarrus <br /> <br />July 1, 2002 through June 30, 2003 <br /> <br />The Older Americans Act required that the service provider attempt to provide services to low- <br />income minority individuals in accordance to their need for aging services. The community <br />Service provider shall specify how the service needs of low-income minority elderly and rural <br />elderly will be met through the services identified on the Provider Services Summary (DOA- <br />732). The narrative shall address outreach and service delivery methodologies that will ensure <br />that this target population is adequately served and conform with specific objectives established <br />by the Area Agency on Aging, for providing services to low-income minority individuals. <br />Additional pages may be used as necessary. <br /> <br />GOAL: To increase participation in services and programs by low-income minority elderly <br />persons at a minimum r.,ate of the percentage of low-income minority elderly in the community. <br /> <br /> Cabarrus County Department of Social Services provides assistance to all individuals <br />requesting services regardless of race with most of our programs serving iow income individuals. <br />The type or extent of services clients receive are based solely on clients' need and availability of <br />funding. <br /> <br /> Through the HCCBG Program, 27% of the clients served are in the minority population. <br />This exceeds the 15.7% minority population of Cabarrus County, Which is reflected in the 2000 <br />census statistics. We will continue to strive to serve the low-income minority population. In our <br />Community Alternatives Program (CAP), 34% of the clients served are in this group. In <br />addition, 32% of the clients served through the In-Home Aide Program had income under 100% <br />of the Federal Poverty Level. <br /> <br /> Outreach by the agency has been accomplished by newspaper articles, brochures, home <br />visits by social work staff, and contacts with other agencies. Since funding is limited for these <br />programs, any new outreach programs will not implement; however, current outreach efforts, <br />which have proven effective in'the past, will continue. Based on current need for services, there <br />are not sufficient funds for a significant increase in the number of clients served. Requests from <br />clients who meet the need for In-Home Aide Services 'will continue to be accepted. New clients <br />will be added as existing clients leave the program or experience a decrease in the number of <br />units needed or additional funding is secured. <br /> <br /> As an agency with funding from several sources, assessments will be made with each <br />case to be sure that funding is' used to best meet the needs of the clients and that Older Americans <br />Act funds are targeted towards low-income minority individuals. <br /> <br /> <br />
