Home and Community Care Block Grant for Older Adults <br /> County Funding Plan <br /> <br />DOA-73; <br /> <br />Methodology to Address Service Needs of Low-income Minodty Eldedy and Rural Elderly <br /> <br /> (Older Amedcan Act, Section 306 (a) (5) (A) (ii) <br /> <br />Community Servic Provider Cabarrus County Planning Services Department <br /> <br />County: Cabarrus July 1, 2002 through June 30, 2003 <br /> I <br />The Older Americans Act requires that the service provider attempt to provide services to Iow-income <br />minodty individusla in accordance to their need for aging services. The community service provider I <br />shall specify how the service needs of Iow-income minority elderly and rural elderly will be met throug <br />the services identified on the Provider Services Summary (DOA-732). This narrative shall address I <br />outreach and service delivery methodologies that will ensure that this target population is adequately <br />served and conform to specific objectives established by the Area Agency on Aging for providing <br />services to Iow-income minority individuals. Additional pages may be used as necessary. <br /> <br />GOAL: <br /> <br />To increase participation in services and programs by Iow income min°dty elderly <br />persons at a minimum rate of the percentage of Iow income minodty elderly in the <br />community. <br /> <br />Objective I. To identify the characteristics of the Iow income monodty elderly and where they are <br /> located in the service area. <br />Strategies: <br /> a. Analyze available data on Iow income minority eldedy clients served. <br /> b. List the pocteds/communities where Iow income minodty elderly live. <br /> c.. Identify minodty organizaitons and other groups who serve Iow income minority elderly. <br /> <br />Objective II. To improve awareness of special needs of Iow income minority elderly. <br /> <br />Strategies: <br /> a. <br /> <br /> b. <br /> C. <br /> <br /> d. <br /> <br />Establish cooperative relationships with minodty organizations in the community, I.e., <br />minodty churches, minority contractors, etc. <br />Educate the Iow income minodty community about available services. <br />Maintain senior center outreach satellites in areas of high concentrations of Iow income <br />minority elderly. <br />Continue the practice of hiring minodty staff. At present, 38% are minority and 58% <br />are Iow income. <br /> <br /> <br />
