PARTICPANTS RESPONSIBLITIES <br /> <br />You have a responsibility to meet with your Social Worker, when requested, to discuss your participation, progress, skills,, interests, <br />and family's needs. <br />You have a responsibility to meet with your Social Worker to develop and monitor your Family & Children's Services Contract. This <br />part of the contract lists the activities you must participate in to help you in becoming self-sufficient. (when your Services ends). <br />The Family & Children's Services Contract also outlines the Services you and your family will receive .in order for you to participate <br />in the Services. <br />You have a ~esponsibility to participate in Services and/or Service-related activities that are listed in your Family & Children's <br />Services Contract to contact your Social Worker monthly regarding your status. <br />You have a responsibility to accept services if it is offered to you in order to become self sufficient. Refusing to accept services or <br />participate in service-related activities may cause your Services to be terminated, unless you have a good reason, <br />You have a responsibility to contact your Social Worker immediately if you have any problems or changes in your situation which <br />make it hard for you to participate in the activities listed in your contract. Failing to comply with {he responsibilities outlined in your <br />contract can result in a delay and cause your Services to be terminated. <br /> <br /> PARTICIPANT'S RIGHTS <br />You have the right to talk with yo~ir Social Worker about your contract. Your contract can be changed if your family's circumstances <br />make it necessary. <br />You have the right to receive the assistance with services that you cannot reasonably provide and that are necessary for you to <br />participate in Services and/or Service Related activities (as listed in your contract) <br />You have the right to request a hearing if you and your Social Worker cannot agree on your Family & Children's Services Contract <br />or if you are terminated. <br /> <br /> <br />
